Hello,
DOH's AO 2016-0014 discusses diabetes-hypertension clubs. I would like to learn more about these clubs, specifically:
- the number of clubs, health care workers, members
- club activities: types, outputs, recordings
- spending on clubs: how much the DOH has spent on these clubs, and what the expenditure was for
- any outcomes or effects of these clubs
- data on monitoring these clubs
- anything more
