Good Day FOI,

I would like to ask request on DOH to update my vaccine record since my 1st boosedoes not yet appear. I will be using for requirements to apply abroad

Here are my details:
Origin: pampanga

1st dose: Astrazeneca 01/11/2022 san fernando pampanga
2nd dose: pfizer 01/27/2023 magalang pampanga
Booster shot: Pfizer 06/09/2023 san fernando pampanga

You asked for Updating my vaxcert.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph  or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
