I am already vaccinated with Yellow Fever & Polio between August to October of 2016. I am an ex crew in a cruise ship & I have submitted my records to my agency. However, they have already lost all my documents since I was unable to go back to the ship for few years now. Also, I am already married now. I would like to ask if I can still get my record to your good office and update my details such as Surname and Civil Status. 

I'm using Armi Jane Mijares, birthdate March 7, 1993 and my address is 108 C. Benitez St. Cubao, Quezon City in your records in case you need it. Thank you so much.
