Good day!
I would like to request a certificate of my vaccination or vaccination card,i have lost my vaccination card and is now . I lost my vaccination card last December. I'm vaccinated in first dose and second dose but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination.
I was vaccinated in KCC mall de Zamboanga, Zamboanga city. My first dose was on November 29, 2021 and the second dose was at December 20, 2021. As I have said above I lost my vaccination card and wasn't able
