August 31, 2022 Dear Carolyn Apao, Greetings! Thank you for your request dated Aug 30, 2022 11:56:53 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Booster shot taken abroad. Response to your request While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. We regret to inform you that we cannot add your booster shot taken overseas in VaxCert. Unfortunately, your vaccination records may not be accessible by the central database which holds the data of recipients of the COVID-19 vaccine from the Philippines. Kindly request for proof of vaccination (i.e. vaccination card/vaccination certificate) from the country where you received your primary dose series or booster shot, provided it has all the necessary information (personal details and vaccination details). Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to DOH Central Appeals and Review Committee, at foi@doh.gov.ph. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
