Good Day FOI,

     I would like to ask request on DOH to update/change the information ( birthdate ) on my vaccine record. Since my birthdate that is recorded is wrong. My real birthdate is January 30,2002. I am hoping that you can read my request. Thank you.

Kindly coordinate directly with the National Patient Navigation and Referral Center (NPNRC) at Telephone number (02)8651-7800 local 2431-2345 or hotline number 1555 local 2 for immediate assistance.

Please prepare your reference number generated thru Vaxcert.doh.gov.ph for verification purposes and ease of transaction.
