Dear Ma'am/Sir,


I would like to ask for your assistance with VaxCertPh because it has been 2 weeks since I requested to update my record for the 2nd dose of Vaccine  to be posted, and until now it has not been updated. I already contacted our LGU and they said that they already upload my Vaccine record last week.


I have an incoming international flight on September 18, 2023 and VaxCert is needed.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Rest-assured that we've already coordinated with Agent 228 from the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. Please expect a call or an e-mail from them, if none, kindly call them back through the number above. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
