Hi, I'm John Edcel L. Guisando. I would like to request a copy of my vaccination card because it was lost on a fire incident. I already got a full vaccination but i only remember the date of my first vaccine that was December 14,2021 but i forgot when i received my second shot. Hopefully you guys can help me out.

Your FOI request is approved. 
About the copy of vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
