To whom it may concern,

Good day! I would like to ask for the following information of yearly surveillance report 2010 to date in Region V (Bicol) only. The information is to aid my research and thesis study.

1. Number of reported cases of water borne diseases such as cholera, typhoid fever, leptospirosis, hepatitis A and schistosomiasis

2. Please also specify the year, gender and age as the disease was reported. Please also include the annual morbidity and mortality rate of the said diseases if possible.
