I would like to raise a concern regarding my vaccination record. I lost my vaccination card recently. I'm fully vaccinated but I don't have other copy of my vaccination card. I was vaccinated in People's Park covered court Valenzuela City.My first dose was on September 16, 2021 and the second dose was on October 14, 2021 and had my booster at Talisay, Batangas on June 20, 2022. As I have said above I lost my vaccination card and wasn't able to take photo of it, but I have a photo of an old VaxCertPH with QR code.
