Good day, I am John P. Anastacio, a masteral student major in Adult Health Nursing at University of the Philippines, and currently enrolled in N-300 (Thesis Writing) with my proposed title of INFORMAL CAREGIVER’S BURDEN, ACTIVITIES OF DAILY LIVING (ADL) AMONG STROKE PATIENTS AND THEIR QUALITY OF LIFE (QoL) IN A RURAL SETTING.

In lieu with this, I would like to request to get a latest data pertaining to the prevalence rate or epidemiology of stroke both morbidity and mortality rate in National, Region V (Bicol) and in particular to the province of Camarines Norte.

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. We already endorsed your data request to the DOH Regional Office 5 in Bicol and they informed us in their letter dated August 15, 2018 that they do not have custody of the data you are requesting.
