May I request a copy or info of my 1st booster  card, because I have lost it. My 1st booster happens on Feb 10, 2023 at Ayala Circuit Makati.

About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
