Hello, I'm writing in behalf of my uncle. He lost his vaccination card and forgot the information since he is a bit old now. Is there a way we can still retrieve a digital vaccination card as we need it here in the hospital?

About the lost vaccination card or vaccination certificate, kindly coordinate
with the LGU where your uncle received his vaccination as their office shall verify your record prior to the issuance of a new vaccination card.
