September 6, 2021 Dear Fred II Rebadulla, Greetings! Thank you for your request dated Sep 05, 2021 12:57:00 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for 9-5-21' Letter from Secretary Duque/Singh-Verherie to Dr. Ho of DPCB-B of Mendiola Siege Peace Armis. Response to your request While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. Please be informed that the DOH has already received an email from you last September 2, 2021 which was already responded. This is regarding your log-in account in the Philippine Registry for Persons with Disability (pwd.doh.gov.ph). We would like to inform you that there has been an update of the system since August 2020. Therefore, the interface of the system has changed. The persons with disability will no longer have to register for a user account. Hence, only the issuing offices will have user accounts. With this, the persons with disability will have to apply online through the "Online Application" of the system, but will still have to proceed to their nearest issuing office (City/Municipal Office) to be given a PWD ID Card. Another option is that they can directly visit their issuing offices as walk-in applicants provided that they bring all the requirements needed for the issuance of an ID card. We suggest that you visit your nearest City/Municipal Office to update your personal information and to be issued an ID card.
