To Whom It May Concern,
We, the 6th year PharmD students of Adamson University, are undertaking a study in partial fulfillment of the requirements of the Doctor of Pharmacy program in S.Y. 2023–2024.
We sincerely ask for your permission to use the leaflet created by Baguio General Hospital and Medical Center as a research tool to assist study participants in quitting smoking. We are determined that this leaflet will be beneficial to our research.
