Magandang Araw po! Pwede po ba maka request ng Vaccination Certificate naisama po kase sa mga nabasa yung aken nung bumagyo po, nakalimutan ko din po yung 1st dose ko kaya di ako makapag fill up dun sa form, salamat po kailangana lang po sa trabaho

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
