I, Chrischell Lucas, a student of Manufacturing Engineering and Management at DLSU, am conducting a research  together with my group and I’m writing to respectfully ask if it’s feasible to request data of stroke and non-stroke patients,  to wit:

History of Stroke (Yes/No)
Gender
Age 
Body mass index (BMI)
Hypertension (Yes/No)
Average Blood Pressure 
Diabetes (Yes/No)
Average Glucose Level
Heart disease (Yes/ No)
Smoking status (Non-smoker/ Light smoker/ Heavy smoker)
Alcohol consumption (Non-drinker/ Light drinker/ Heavy drinker)

We regret to inform you that the Field Health Services Information System (FHSIS) Unit of the Epidemiology Bureau does not collect data on the said request but we are sending the list of Notifiable Diseases available in the FHSIS. All data being collected by the FHSIS can be found at this link: https://doh.gov.ph/publications/serials
