Good day,

I would like to cancel my update request of my vaxcert because I thought that my second dose was not recorded. However, it was my fault to fill in the wrong date on my second dose therefore my second did not come out when I did my certification of vaccine. On my behalf I apologize for it. I hope you understand and have a nice day ahead.

You asked for Cancellation of Update Request of VaxCert.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
Rest-assured that we've already coordinated with Agent 212 from the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. Please expect a call or an e-mail from them, if none, kindly call them back through the number above. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
