Hello,

I would like to request a file or copy of records if possible for my daughters babys book immunization record. we lost most of our documents during the flood happened in Iligan City last 2017. and i need my baby's immunization record for immigration purposes and medical purposes. I am hoping for your kind consideration of my request.

We regret to inform you that we do not have custody of the immunization record and ask that you coordinate directly with the health center where your child  received the vaccination for the record.
