Response to your request

While our aim is to provide information whenever possible, in this instance the DOH EPIDEMIOLOGY BUREAU does not have the information you have requested. As per advised, this is handled by the HOSPITAL PATIENT SAFETY RECORD and for Internal Consumption only.  However, you may wish to contact a specific Hospitals (PRIMARY, SECONDARY & TERTIARY) to coordinate with them regarding this concern.
