Dear FOI,

I  would like to have my Vaxcert updated in the system as my second dose is not recorded. I will be needing the vaxcert to travel local and abroad.

Below is my vaccination details:

Malasiqui, Pangasinan

First dose( Pfizer) - 10/18/ 2021
Second dose( Pfizer)- 11/18/ 2021 ( no record)

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. If your vaccination record is
unavailable, the record may not have been
uploaded yet in the Vaccination Information
Management System (VIMS) by the vaccination
center.
For further assistance, you may contact the Vax Cert

Kindly coordinate also with the LGU where you
received your vaccination and request for
assistance to generate your vaccination
certificate.
