I would like to request correction of my first name in my vaccination record and certificate on vaxcertph. 
The website / vaxcert record for my last name is Jenebeth H.Batocael as my last name,But the correction in my last name or my exact last name is Jenebeth Hibaya ,I really need to change that.Asap Po sir/maam.Thank you po .

Your FOI request is approved. 

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate with the Manila Health Department under the Manila LGU where you received your vaccination and request for assistance to correct your vaccination record and certificate. Thank you.
