Hello po magrequest po sana ako ng copy po ng vaccination card ko po nawala po kasi nung nag-ooperate po kami sa bundok bale sundalo po ako. Nakalimutan ko na din po kung anong date yung kung kailan po yung vaccine shot po. Salamat po sana po mapansin. Kailangan po namin as requirements for schooling po

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
