I would like to raise a concern regarding my vaccination record. I lost my vaccination card I'm fully vaccinated based on the Pasig record https://pasigpass.pasigcity.gov.ph/PasigHealth

I was vaccinated in Brgy Sagad, Pasig City last 2021

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
