would like to raise a concern regarding my vaccination record. I lost my vaccination card recently. I already had my first booster shoot but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. 

I was vaccinated in Tacloban City at their City Health.I was vaccinated last August 18,2924. As I have said above I lost my vaccination card and wasn't able to take photo of it, but I do have picture of myself during the vaccination and I have the vaccination number.

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
