Hello Ma'am/Sir. I hope you are well.

I would like to request an updated Vaxcert in the system as my 1st and 2nd booster is not recorded. 
I will be needing the updated vaccination certificate to travel abroad.

First dose: City of Talisay, Negros Occ.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
