I recently discovered that my COVID-19 vaccination certificate is missing, despite completing my first, second, and booster shots at Kinoguitan, Misamis Oriental. Additionally, as far as I can remember, my first and second dose certificate contain incomplete information; my full name, Ernest Gerard B. Durano, appears as only Ernest Durano. I am concerned about the accuracy of the records and would appreciate assistance in obtaining a replacement certificate with the correct details.
