Good day,

The expected dataset is top 5 diseases as diagnosed per location with the number of cases. An alternative could be the number of diseases per major disease groups: Infectious Diseases, Deficiency diseases, Hereditary Diseases (genetic and non-genetic), and Physiological Diseases per location with the number of cases as well. I hope this clarifies the request. Otherwise, please let me know if further clarification is needed.

While our aim is to provide information whenever possible, in this instance this Office does not have  the information you have requested. 

Visit the FHSIS Annual Reports posted in the DOH website at https://doh.gov.ph/publications/serials

The Field Health Services Information System (FHSIS) reports here in the Central Office only capture data in the regional, provincial, and highly urbanized cities. Data for the barangay level is not available in our published FHSIS reports. Kindly refer the eFOI request to the CHDs and MOH-BARMM (contact details found at the DOH website:  https://doh.gov.ph/regional-office-directory), which may be able to provide the specific data he needs. All data being collected by the FHSIS can be found at this link: https://doh.gov.ph/publications/serials.
