Hi po ..San po b ko mgrerequest ng record ng immunization?..pinabalik balik lng po ako sa commonwealth health center..tas pinapunta p po akong archive,kc dun daw po ako kukuha..pinuntahan ko  po pero Sabi nla Hindi daw dun  kuhaan, Sana po matulungan nyo  po ko.. salamat

Response to your request

While our aim is to provide information whenever possible, in this instance, we regret to inform you that we do not have custody of immunization record. We ask that you coordinate with the Commonwealth Health Center  where your child received the immunization for this request
