hi! Ako po si angelito, mondilla oraye. Request lang po ako ng official copy ng vaccination card, nawala po ang aking vaccine card noong december 2022,  naganap po ang bakuna sa akin sa lugar po ng cabuyao laguna, centro mall.  Thank you!

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
