I would like to raise a concern regarding my vaccination record. I lost my vaccination card this day January 4 , 2024. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my 1st dose vaccination. 

I was vaccinated in Lucena City. My first dose was on December 31, 2021 and the second dose was at January 31, 2022. As I have said above I lost my vaccination card and wasn't able to take photo of it.

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
