Hello I would like to request change of my Middle Name meron kasi nkalagay na Rebuldela instead na wala po dpat akong middle name. Need lang po na match ang vax cert details ko seaman’s book ko po and Birth Certificate,I am a seafarers malapit na kasi alis ko sa aug 18 po.thanks!

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
Rest-assured that we've already coordinated with Agent 180 from the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. Please expect a call or an e-mail from them, if none, kindly call them back through the number above. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
