Hi i would like to request  . 
my first dose on my vax cert is not recorded.
yet my 2nd dose has a record on my vax cert. i just really need to because im applying work abroad thanks.

Response to Request

Your FOI request is approved. 

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. 

kindly coordinate with the LGU where you received your vaccination and request for assistance to update the details in your vaccination certificate. The details of your vaccination certificate should be the same as the vaccination card. (date and place of vaccination)

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
