Good day! I would like to request a copy of my child's immunization record. It's one of the requirement in enrolling for day care. I lost it during the flood in our Brgy. I hope I could get a copy.

While our aim is to provide information whenever possible, in this instance, we regret to inform you that we do not have custody of barangay immunization record. We ask that you coordinate with the Municipal Health Center  where your child received the immunization
