You asked for IMMUNIZATION RECORDS.

Response to your request

We regret to inform you that DOH does not have custody of immunization records and we ask that you coordinate with the Barangay Bugo, Cagayan de Oro City, Misamis Oriental Barangay Health Station (BHS) for this information.

In addition, we cannot  disclose medical and immunization records as this eFOI portal is a public website and this  is contains sensitive personal information covered under the Data Privacy Act of 2023
