Hi! I would like to inquire where can I take the following:

1. Booster Shot Vaccination
2. Yellow Fever Vaccination
3. Polio Vaccination 

These vaccination are required by the agency I am trying to apply, in order for me to be accepted an become a seaman.
Please help me, any information what locations can I take these vaccinations, within Manila. Thank you for your reply.

Response to Request

Your FOI request is approved. 

For booster shot vaccination, kindly coordinate with your nearest LGU to avail of the vaccination. Refer to the list of COVID-19 vaccination sites on the DOH website at https://doh.gov.ph/covid-19-vaccination-sites

For Yellow fever and Polio vaccination, please coordinate with the Bureau of Quarantine at https://icv.boq.ph/ and book for an appointment for the services
