Good day to your good office.

I would just like to ask for kind assistance regarding the request for the information update/correction of my vaccination record. Kindly see the details of the concern below. 

1. Name correction - My first name is "Cesar Jr." With dot (.) but in the record, "Cesar" only appears. 
- CESAR JR. GUTANG GALLAMORA (Correct one)

I have already done the request for update in the VAXCertPH site last June 12, 2023 and sent an email in the vaxcertsupport@doh.gov.ph attaching my IDs and Affidavit to support the request last June 19, 2023.

You asked for Covid19 Vaccination Certificate and Covid Vaccine.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
