Good day! My name is Iris Jane Mangente, a 5th-year BS Architecture student at Mapua Malayan Colleges Mindanao. I am writing to request an endorsement for me to conduct an ocular visit to the following Hospice Care in the Philippines:
1. Madre de Amor Hospice Foundation
2. Ayala Alabang Hospice Care Foundation
3. De La Salle Hospice Care 
4. FEU-NRMF Medical Center Hospice & Palliative Care Service
5. Makati Medical Center (Integrated Palliative and Home Care Unit)

We regret to inform you that DOH does not provide endorsement for the conduct of ocular visit for your study. Furthermore, we would like to inform you that the hospice care facilities that you indicated are not DOH Hospitals. We ask that you coordinate directly and write a letter request to these hospice care facilities duly signed by your professor on your request for ocular visit.
