Hi I would like to request for update of my Vax card that doesn't have a 2nd dose record below are my details and I want to add my second dose  on the Vax card since I need it on my job.

1st dose Pfizer 
Date: 11-19-2021
Place: Cajidiocan, Romblon

2nd dose Pfizer 
Date: 12-10-2021
Place: Cajidiocan, Romblon 

The 2nd dose is missing on the Vax card and I want it to update, thank you.
