Hi! I would like to request to correct the givenname and suffix in my Covid-19 Vaccination card. The name listed is Jonathan M. Paga. Since I have a problem with my PSA certificate because it has many errors such as the middle name and etc.
So we decided to cancel it and us my other name neil mercado paga jr. My case is that I have double registered name in  PSA and we decided to cancel the Jonathan B. Paga and Use my updated name which is Neil Mercado Paga Jr. I will be needing the correct card for my vaxcert for travel purposes. Kindly let me know how this issue can be resolved.
