Hi I would like to request a digital copy of my childhood book or immunization record as it is needed for my employment here in New Sealand.

My family couldnt find my yellow card back in Philippines as it is probably washed out from previous storms/floods.

While our aim is to provide information whenever possible, in this instance, we regret to inform you that we do not have custody of immunization record. We ask that you coordinate with the Barangay Health Center/Rural Health Unit  where your child received the immunization for this request.
