I would like to ask request on DOH to update my vaccine record since my booster shot same as first and second dose does not yet appear. I will be using the details to travel abroad. 

Here are my details:
Origin: Tarlac City 

1st dose: sinopharm 09/06/21 Tarlac City 
2nd dose: sinopharm 09/27/21 Tarlac City 
Booster shot: Moderna 03/07/22 Tarlac City

You asked for Vaccine certificate.

Response to Request

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Don't worry, I already contacted Agent 212 from the Vaxcert Team and requested for assistance to generate your vaccination certificate. Please expect her to call you on your phone number or you can call them at (02) 8651-7800 local 2432.
