You asked for Annual Health Facility Statistical Report.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested. This is the same as your eFOI request  #DOH-722850324544 that was denied. Our DOH CALABARZON CHD (DOH Regional Office 4A) is advising you to directly coordinate with the target hospital to retrieve the data being requested. We shall be forwarding the response from our regional office to your email for your reference.
