Good day! I would like to raise a concern regarding my vaccination record. I lost my vaccination card yesterday, September 06, 2023, around 6pm and unfortunately, I don't have a record of it. I was vaccinated in Vista Mall, General Trias last year, in the 3rd or 4th week of November 2022. As I have stated, I lost my vaccination card and wasn't able to take a photo of it. Can I request for a proof of vaccination or a record detail of my vaccination? Thank you!
