Good day! 

I would like to make a request of copy of my immunization record since baby. My mother misplaced the book and nowhere to be found. I needed the copy for my application abroad. I am residing at Tipacan, Lipa City Batangas. Thank you!

While our aim is to provide information whenever possible, in this instance, we regret to inform you that we do not have custody of immunization record. We ask that you coordinate with the Lipa Health Center  where you received the immunization for this request
