Dear FOI,

I  would like to have my Vaxcert updated in the system as my second dose is not recorded. I will be needing the vax cert to travel locally and abroad.

Below are my vaccination details:

City of MAKATI, NCR fourth district

First dose (Sinovac) - 08/13/ 2021 (recorded)
Second dose (Sinovac)- 09/10/ 2021 (no record)

You asked for Information for 2nd Dose Missing on Vaxcert.

Response to Request

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

If you want, you may contact the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2432-2433 or kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
