Hi! I would like to request a vaccine card because mine got lost. I got my first dose at July 28, 2021 in Bicol Medical Center in Naga City with the Brand Johnson & Johnson and my booster dose at February 10, 2022 at Iriga City Hall. I hope my request gets fulfilled for I will be using it for a trip in Manila next week.

About the lost vaccination card, kindly coordinate directly with the Bicol Medical Center and Iriga City Hall where you received your vaccination as their office shall verify your record  prior to the issuance of a new vaccination card.
