vaccination record. I lost my vaccination card this last last year.  I was vaccinated in Lakefront Taguig City , I dont remember the exact date when was my 1st and 2nd dose . As I have said above I lost my vaccination card and wasn't able to take photo of it. I have no more access with my email and phone number . But i only can give was my basic information Name : Maria Flerida II Aguilar Deima Birthday: October 25 1999 
I need a copy for my work requirements . I really that you can help me . Thank you

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
