Hi I would like to request for information regarding to my lost vaccination card/id. I lost my vaccination card/id this August 2023. I have a picture of my card/id and other proof that I am vaccinated. I was vaccinated at Purok 10 upper Tugbok, Davao City Davao del Sur. My 1st dose was on September 02,2021 and 2nd dose was on September 30,2021. As I have said I have a pic of my vaccination card/id, from front to back. Hoping for your help thank you.

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
