Good Day!

I recently lost my vaccine card this August 2023 and I was wondering if I could request for a new one.

I was vaccinated with Pfizer and got my first dose on November 9, 2021 and my second dose was on November 30, 2021. Both were taken in New Era General Hospital. I have photos to verify this.

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
